奧迪慈善基金（下稱「本會」）致力遵守《個人資料（私隱）條例》的規定，確保所儲存的個人資料得到充份保護，準確無誤，並有妥善的儲存方法。

Otic Foundation (hereinafter "Otic") is committed to complying with the provisions of the Personal Data (Privacy) Ordinance, so as to ensure that all personal data collected is secure, accurate and properly stored. 

收集資料目的及使用準則

1. 本會將循合法和公平的途徑收集個人資料，並僅用於與本會服務和活動有直接關係的合法目的。

2. 在合理可行的範圍內，本會在收集資料前會通知資料當事人收集資料的目的，並將依據所述目的使用該等資料。

3. 資料當事人向本會提供個人資料純屬自願，如未能提供足夠資料，本會可能無法有效處理申請或提供相關服務。

4. 本會可能使用資料當事人的個人資料 (包括但不限於姓名、電話、傳真、電郵、郵寄地址、所屬機構)，以便於以下用途：

• 與資料當事人通訊；

• 處理報名/申請；

• 發行收據；

• 進行研究/分析/統計；

• 處理捐款/籌款；

• 義工招募/安排；

• 收集意見；

• 發送活動邀請/推廣資訊；

• 處理與本會相關之項目事宜。

Purpose of Data Collection and Terms of Use 

1. Personal data shall be collected by lawful and fair means, and shall be used for lawful purposes directly related to the services and activities of Otic.

2. As far as is reasonable and practicable, Otic will inform the data subject, on or before collecting personal data, of the purposes for which the data is collected, and will use such data only for the stated purposes.

3. Personal data is supplied by the data subject on a voluntary basis.  If insufficient data is provided, Otic may not be able to process the relevant application or provide the relevant services.

4. Otic may use the personal data supplied by the data subject (including but not limited to name, phone number, fax number, e-mail address, postal address, name of organization) for the following purposes:

• for communications with the data subject;

• for handling applications;

• for issue of receipts;

• for conducting research / analysis / statistics;

• for promoting, soliciting or handling donations / fundraising matters;

• for recruitment or work arrangement of volunteers;

• for surveys / opinion;

• for issuing invitations / promotional materials;

• for any other incidental matters relating to Otic. 

個人資料查詢或更改

根據《個人資料（私隱）條例》，資料當事人有權查閱及修改個人資料。如欲查詢、更改或刪除個人資料，請以書面形式通知本會：

Enquiry or Update of Personal Data

Pursuant to the Personal Data (Privacy) Ordinance, a data subject must be given access to his/her personal data and be entitled to request corrections.  For enquiries, update or deletion of personal data, kindly inform Otic in writing:
